Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling into an elevator shaft and/or being caught between ascending or descending elevator car and shaft wall due to the fact that an electromechanical interlock for a hoistway door was not present. a)East elevator, 4th floor; on or about 8/15/95. Among others, one feasible and acceptable means of abatement would be to install a hoistway door electromechanical interlock as detailed in American Society of Mechanical Engineers A.17.3 - 1993 2.7.1.
