Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to the hazard of fire and explosion (as occurred on March 7, 1991) in that the employer did not take adequate measures to prevent a hazardous release of flammable vapors/gases (such as, but not limited to, methane and other hydrocarbons) into the engine room. Employees performing assigned duties to exchange compressor valves #3 and #4 of enigne #3 with compressor valves #3 and #4 of engine #4 were not provided a means for preventing/controlling compressor leakage of toxic/flammable gases. Among other methods, one feasible and acceptable method to correct this hazard and protect employees would be, to provide a means to purge with an inert gas and isolate gas compressors and related piping.
