29 CFR 1926.251(a)(1): Defective rigging equipment was not removed from service: The employer does not remove defective equipment from service. This violation occurred on or about June 2, 2015 and times prior thereto, at or near 5610 W. Davis St. in Conroe, TX, where employees were exposed to a struck-by hazard when lifting 23,000-pound concrete culverts with a defective lifting hook. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that defective rigging equipment is removed from service.
