Section 5(a)1 of the Occupational Safety and health Act of 1970: on or about 11/24/93 the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that: employees were exposed to the hazard of being struck by dislodged hoisting equipment. a) Shaft 19b on 11/24/93. The employees engaged in the construction of the water tunnel number 3 shaft 19b were exposed to the hazard of being struck by falling hoisting equipment, in that the anchorages (hold down rods) were not installed to prevent overturning during use as specified in Economy Forms Drawing DSME - 014. The New Era Winch used during the concrete construction of shaft 19b was positioned on the A-10 roll back platform which was fully extended over the shaft. The wire rope cable of the winch was attached to the concrete formwork in the shaft and was being pretensioned without the use of the hold down rods to prevent the winch and platform from overturning. Among other methods, one feasible and acceptable abatement method to correct this hazard is to anchor the hoisting
