Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flying part(s) hazard: (a) On or about November 11, 1994, employees were exposed to being struck by the #1 right jacket water header after it was forced out of it's Dressler coupling pipe joint on the Cooper Bessemer Z330 engine located in Compressor Building #3. This exposure occurred when the head of the #1 right cylinder had been removed and the jacket water system was pressurized to check for leaks. The #1 right cylinder head had served as an anchor for the pipe joint and because of its removal the end of the water header was subject to being forced out of the pipe joint once the system was pressurized. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Ensure cylinder heads are in place when the system is pressurized to check for water leaks in the #1 right and the #2 left cylinders of the Cooper Bessemer Z330 engine, located in Compressor Building #3, or implement equivalent method to ensure pipe joints are anchored.
