Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. DOW Rack - Four of the eight bolts were missing at each of the flange couplings at both sides of each of the eight filters for the 18 to 21 psi solvent lines, on or about 3/7/95. b. Dock Lines by the Truck Scale - Four of the eight bolts were missing in at least two 10-inch diameter pipeline flange couplings along the 60 to 80 psi piping system for Conoco No. 2 diesel fuel, and four of the eight bolts were missing in at least two 10-inch flange couplings along the 60 to 80 psi piping system for ICI Americas Inc. TRIKLONE N, on or about 3/8/95. Among other methods, one feasible and acceptable abatement method to correct this hazard is to require employees to use the appropriate number of bolts in accordance with the manufacturer's pipe flange design.
