Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed: a)Starboard side of USNS NIAGARA FALLS, pierside: An employee was exposed to the hazard of being struck by a load caused by the improper spooling on the main hoist drum, corroded hydraulic lines and the operator's inability to observe the load through broken glass in the window of the cab. One method of abating the hazard would be to follow the requirements of ANSI Standard B30.5 1968 Crawler, Locomotive and Truck Cranes section 5-2.3.3 c. which requires that repairs or replacements shall be provided promptly as needed for safe operation.
