Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)First Hawaiian Bank: Employee operated a Komatsu PC200 at an excavation 230 feet long by 28 feet wide by 10 feet deep. Bolts and nuts were modified and used as pins for the shovel, instead of the pins originally provided by the manufacturer to secure the shovel. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD: 1.To follow the American Equipment Manufacturers (AEM) "Safety Manual for Operating and Maintenance Personnel" which requires that damaged and missing parts are replaced, and 2.To follow the Manufacturer's Operator Manual by using only service parts meeting manufacturer specifications, and 3,To train operators and maintenance personnel to inspect equipment for defects and to complete service and repairs before equipment is placed back in service.t
