Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1)of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Employee(s) required to enter a cement hopper at the batch plant on or about July 27, 1988 were exposed to the hazard of engulfment by caked and loose cement powder. One feasible and acceptable abatement method, amond others, to correct this hazard is to develop and implement a permit entry confined space program which contains, at a minimum, the following elements: 1) Identification of each permit entry confined space. 2) Providing affected employee(s) with specific training prior to authorizing entry. 3) Assuring availability and used of all protective clothing and other personal protective equipment necessary for safe entry. 4) Assuring ready availability of rescue and safety-related equipment and services, such as lifting or retrieval devices and others, necessary for the entry. 5) Providing for and requiring the use of retrieval lines. 6) Determining and evaluating potential of atmospheric contamination. 7) Provision of an attendant for each permit entry. 8) Establishment and implementation of an entry permit system that identifies and defines roles of persons who must sign the entry permit.
