29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards: a) On May 6, 2015, at Section 11, Township 4S Range 7E, Bungay IL, one employee was working while on the board on the derrick, 55 feet high without fall protection with a retractable lanyard. b) On May 6, 2015, at Section 11, Township 4S Range 7E, Bungay IL, one employee climbed the ladder to the derrick board area, 55 feet high, without using fall protection attached to the ladder safety cable.
