Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Warehouse Number 5, Dock B ramp - Employee(s) operating the 83 model S80XL HYSTER industrial truck (Serial Number D004V01566J Type LP) equipped with a Model SQ8PHS4BQ Long Reach Manufacturing Company box clamp, was not using the seatbelt, on or about 12/14/92. (b) Warehouse Number 5, Dock B Ramp - Employee(s) operating the Number 92 Model S80XL HYSTER industrial truck (Serial Number D004V05227N type LP) equipped with a Model SQ8PHS4BQ Long Reach Manufacturing Company box clamp did not use the seat belt, on or about 12/12/92. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Ensure that employee(s) utilize seat belts while operating industrial trucks.
