Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On and before June 3, 1988, at the Killian Shopping Center Project, wooden roof trusses being erected were not adequately braced to prevent a truss collapse, nor were the trusses properly stored; exposing employees to the hazards of being struck by or falling from collapsing trusses: The following deficiencies were found: 1) None of the requirements of B.W.T.-76 for temporary bracing were followed: a) Number of lateral braces insufficient b) No diagonal bracing used c) All lateral braces were not lapped a minimum of two (2) trusses d) Lateral trusses were not continuous for entire length of the buidling e) Web and bottom chord bracing were insufficient in amount f) Lumber used was not minimum 2 x 4 material g) Nails used were not double headed 16 penny each h) Plywood bundles of more than eight (8) sheets for each pair of trusses were stacked on the trusses i) Trusses were not stored level causing lateral bending which loosened splice plates j) Stored trusses were not blocked every 8 to 10 feet of length. Among other methods one feasible and acceptable method of abating these hazards is to install trusses in accordance with the requirements of the Truss Plate Institute which are contained in BWT-76 "Bracing Wood Trusses".
