Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards: a) On or about May 5, 2015, employees were exposed to being struck-by and/or crushed by pallet racks while working in and around the 47C pallet rack where the pallet racks were not anchored and bolted to the ground. b) On or about May 5, 2015, employees were exposed to being struck-by and/or crushed by pallet racks while working in and around the 48C pallet rack where the pallet racks were not anchored and bolted to the ground. c) On or about May 5, 2015, employees were exposed to being struck-by and/or crushed by pallet racks while working in and around 42C pallet rack where the pallet racks were not anchored and bolted to the ground. d) On or about May 5, 2015, employees were exposed to being struck-by and/or crushed by pallet racks while working in and around area 41C pallet rack where the pallet racks were not anchored and bolted to the ground. Among other methods, one feasible and acceptable method to correct the hazards noted are to follow the manufacturer installation instructions and/or conform with installation and assembly instructions and the American National Standards Institute (ANSI) standard MH16.1-2012 (a revision of MH16.1-2008): Specifications for the Design, Testing and Utilization of Industrial Steel Storage Racks, which includes, among others, the following: a. Section 1.4.7 Column Base Plates and Anchors: The bottom of all columns shall be furnished with column base plates, as specified in 7.1. All rack columns shall be anchored to the floor with anchor bolts, which shall be designed in accordance with Section 7.3, to resist all applicable forces as described in Section 2.1 or Section 2.2. b. Section 1.4.2 Plaque: The owner is responsible for displaying in one or more conspicuous locations permanent plaque(s). Each plaque shall have an area of not less than 50 inches. Plaques shall show in clear, legible print (a) the maximum permissible unit load and/or maximum uniformly distributed load per level, (b) the average unit load, and (c) maximum total load per bay.
