Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: a) Fire and explosion hazards that may result from leaking flammable/combustible liquids from improperly assembled high pressure flanged joints in the Alcohol Department. Among other methods, one feasible and acceptable method to correct this hazard is: 1) Establish written disassembly inspection and assembly procedures for high-pressure joints. These procedures are to include specifications for inspection of threaded components, torque requirments, final inspection and pressure testing. 2) Insure that employees performing work on high-pressure lines and joints are trained in the above procedures.
