Text For Citation: 01 Item/Group: 001 Hazard: CUTS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: "L" bay, west exterior, on 5/23/91, employees were exposed to the hazards of severe cuts, broken bones and/or death while performing work on, under or between an empty derailed car without the use of a blue signal or other warning device. The hazards can be abated by, but not limited to a program including the following criteria: (a) Establish written procedures when performing work on rolling equipment on tracks and train employees in these procedures. (b) Display a blue warning device at each end of rolling equipment and/or at each manually operated switch providing access to that track. (c) Assure that other rolling equipment does not reduce or block the view of the blue signal. (d) Assure that all manually operated switches to tracks are locked with an effective locking device and shall not be removed unless informed by the person in charge that it is safe to do so. (e) Document enforcement of the established procedures and training.
