OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees performing well treatment operations: On or about April 23, 2015, and at times prior thereto, during well treatment at Lime Rock Resources, Norge Marchand Unit No. 32-2, SE NW Sec 23-7N-8W, Chickasha, OK 73018, in Grady County, the employer did not ensure that all operating procedures were fully documented when treating a high pressure well. Among other methods, one feasible and acceptable abatement method is to fully document all well treatment operating procedures in writing. Abatement documentation required for this item.
