Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free of hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by mobile heavy equipment (e.g. front end loaders) while walking as pedestrians/working in and around blind corners associated with plant kiln doors: On or about April 21, 2015, employees were exposed to being struck-by Caterpillar Front End Loader (Model # 924G) while employees routinely checked combustion air ports every 15 minutes. The front combustion air ports on both sides of the kilns are located in blind spots made worse when the doors are opened and front end loaders are filling the kiln with wood.
