29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about April 23, 2015, the employer failed to ensure employees cleaning chips from the AmeraSeiki CNC, model number VB-2, serial number, 2010028 utilized the company's lockout/tagout procedures to lock out the equipment . b. On or about April 23, 2015, the employer failed to ensure employees cleaning chips from the Phoenix CTC Copying Milling Machine, model number VTC-30-10, serial number 131056, utilized the company's lockout/tagout procedures to lock out the equipment.
