29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board. On or about April 3, 2015, VCNA Prairie, Inc. did not maintain a standard railing adjacent to the mixer dump hopper that is a permit required confined space. The mixer dump hopper wall adjacent to the access platform extended 1.65 feet vertically above the platform surface. Employees were exposed to a fall hazard of at least 17 feet into and through the mixer dump hopper. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
