29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 211 South Grove Street: On or about March 20, 2015 and at times prior, employees were exposed to an excavation cave in while installing a new four inch sewer line without adequate training in the recognition of excavation hazards and the means to control these hazards. b) 1325 River Street: On or about March 31, 2015 and at times prior, employees were exposed to an excavation cave in while installing a new four inch sewer line without adequate training in the recognition of excavation hazards and the means to control these hazards.
