29 CFR 1926.454(b): The employer did not have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: On a tubular scaffold, on or about April 7, 2015, employees were exposed to fall hazards while performing stucco work on a 5 tier scaffold that was missing guardrails, planks, cross braces, and a ladder for access when the scaffold was not erected by a person trained to recognize hazards.
