OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught-between hazards. a. On or about April 3, 2015, located throughout the facility, employees operated approximately twenty-seven Crown 5500 series RR forklifts while being exposed to caught-between hazards, due to the forklift's open operator compartments operating near the horizontal intruding hazards from the racking systems.
