29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about March 24, 2015, employees were exposed to cave-in hazards while working in a trench in excess of 6 feet in depth where there was no protective systems provided. The employer was previously cited for this occupational safety and health standard or its equivalent, 29 CFR 1926.652(a)(1) on January 29, 2009 with respect to a worksite at 6th & Cottonwood, Las Animas, CO 81054.
