29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) At 3459 Gulf Shores Parkway Gulf Shores, Alabama: On or about March 25, 2015 and at times prior; the employer failed to ensure employees working from an elevated aerial lift basket used a fall harness and lanyard exposing employees to a fall hazard of 23 feet to the ground below.
