29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) On or about April 8, 2015, during maintenance activities in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, an electrical maintenance technician performed hazardous energy control procedures(lockout/tagout) on the Tesker Thread Roller number three. The employee had not been trained by the company as an authorized employee to recognize all applicable energy sources, which include electrical and hydraulic, and the methods and means to isolate those energy sources. b) On or about March 24, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, the Tesker Thread Roller number three and six operator performed thrust bearing greasing operations and did not utilize hazardous energy control procedures(lockout/tagout). The machine operator had not been trained by the company as an authorized employee to recognize all applicable energy sources, which include electrical and hydraulic, and the methods and means to isolate those energy sources. c) On or about March 3, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, the Tesker Thread Roller number three the operator performed thrust bearing greasing operations and did not utilize hazardous energy control procedures(lockout/tagout).
