29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: Jobsite at 505 Washington Street, Boston, MA Employees were exposed to trip and fall hazards from un-guarded/un-covered floor holes and floor hole covers that were not secured on all 11 floors of the jobsite, where a competent person did not adequately inspect the site to ensure that hazardous conditions did not exist.
