29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. Building 126 - Employees removing snow and ice from the buildings roof were not protected from falls of 26 feet 5 Inches when fall protection equipment was not used or provided. Sequel Development & Management, Inc. was previously cited for a repeat violation of an equivalent occupational safety and health standard, 1926.501(b)(13), which was contained in OSHA inspection number 1016106, citation number 002, item number 001B which was issued to the employer on January 17, 2015, with respect to a workplace located at 124 Mammoth Rd., Hooksett, NH 03106 where employees working from the roof were exposed to a fall of approximately 27 feet. Sequel Development & Management, Inc. was previously cited for a violation of an equivalent occupational safety and health standard, 1926.501(b)(13), which was contained in OSHA inspection number 898943, citation number 001, item number 003 and was affirmed as a final order on June 11, 2013, with respect to a workplace located at 10 Woodview way, Manchester, NH 03102 where employees were exposed to falls between approximately 10 and 30 feet from unguarded roof edges, wall openings, window openings, floor openings, floor edges and leading edges.
