Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer is failing to protect employees from potential hazardous working conditions associated with the construction repairs to an elevator leg without having verified through inspection or an inquiry of inspection records that it was stable prior to climbing the access ladder. The most recent example of this was found on a residential construction located at 401 East Front Street, Hershey, NE 69143 where the employer had an employee climbing the access ladder on the site elevator leg as means of access to disconnect and reattach the down tube from the leg; the south support cable connection point on the leg broke loose resulting in the legs collapse pinning the employee inside the ladder cage between the leg and the roof ridge on the adjacent structure.
