Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards resulting from the improper use of a fiberglass stepladder: a) 2870 East Allegheny Ave., Philadelphia, PA 19134 - Employees were exposed to fall hazards while climbing a fiberglass A-frame ladder in the closed position. The ladder was leaning against the metal structure of a sand hopper and was used to access the top of the hopper during the installation of a 2600 pound metal grate. The employer did not ensure that the ladder was used in the open position with all four legs set on a firm level surface, on or about February 13, 2015. One feasible method to correct the hazard includes, but is not limited to: 1) Follow the manufacturer's safety instructions for climbing and use of a ladder. 2) Train or retrain employees in the use of ladders.
