AmherstMadison, Elk River behind Women's and Children's Hospital: On or about 2/11/15, employees used the Dewalt angle grinder with a 4 1/2" abrasive wheel without the use of a safety guard. ABATEMENT CERTIFICATION IS REQUIRED.
