Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of being struck by pressurized equipment: Employees were exposed to the hazards of being struck by pressurized equipment (ball valve & connections) which was inadequately sized and had been subjected to pressure of approximately 2,500 p.s.i. One feasible and acceptable abatement method, among others, to correct the hazards is to assure that all pipe fittings, valves, and unions placed on or connected with blowout prevention equipment, well casing, casinghead, drill pipe, or tubing and exposed to well pressure shall have a working pressure rating equal to or greater than the maximum anticipated well surface pressure as specified by API RP54 92, page 11, paragraph 5.4.3. This alleged condition occurred on or about January 6, 1995.ry
