Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: %% ) On or about April 24, 1992, am employee was building a 16 inch ight truck tire on NANCY #13 85 B tire building machine in Dept. 74, when the bead wire separated from the bead during the process, xposing the employee to the hazard of being hit by the rotating ead wire, and/or pulled into the machine by a bead wire loop. %% ) On or about April 28, 1992, am employee was building a 16 inch ight truck tire on OSCAR #4 85 B tire building machine in Dept. 74, when the bead wire separated from the bead during the process, xposing the employee to the hazard of being hit by the rotating ead wire, and/or pulled into the machine by a wire bead loop. %% mong other methods, feasible and acceptable abatement methods to orrect this hazard may include, but not necessarily be limited to he following: %% . Beads on tires will be all double wrapped; %% . Modifications will be made to the chuck and stitcher on the bead machine; %% . Develop Standard Operating Procedures (SOP's) for all tire building machine operators with daily reports on machine conditions/problems to management; %% . Engineering changes will be made on the drums of the tire machine to eliminate improper spotting of splice; and %% . Training on safe operation of tire machines for all operators of such machine with annual review. %%
