Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a).....the hazard of being crushed between the gate and Personnel elevator. At Building #5/10 on several floors, employees were exposed to a pinch point hazard due to the inside and outside elevator gates being only 5' high which would allow body parts to be placed over the gate while the elevator is in motion. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Install gates that are 6' in height. b).....the hazard of being crushed by a personnel elevator. At Building #5/10 on the 1st floor, employees were exposed to a crushing injury due to the outside elevator gate of the personnel elevator was free hanging and could be pushed inward into the shaft. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Correct the damaged door by placing it back on track. 2. Inspect the elevator on a regular basis. c).....the hazard of being crushed by a personnel elevator. At Building #5 on the 1st level, employees were using a personnel elevator that had an outside solid door that could be opened when the elevator was not in position on the floor. Among other methods, one feasible and acceptable means of abatement is to: 1. Correct the micro-switch in the door that no longer is working. 2. Inspect the elevator on a regular basis. d).....the hazard of being crushed between the personnel elevator and concrete floor. At Building #20 at the west elevator, employees were exposed to a pinch point hazard due to the inside gate of the personnel elevator having a 12" & 34" hole Among other methods, one feasible and acceptable means of abate- ment is to: 1.
