Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Shipping Dock Area, North Wall - employees were using a make shift double leg wire rope sling on the Whiting floor operated crane, 6 ton to lift an empty hopper weighing approximately 1,000 lbs. and a full hopper weighing approximately 3000 lbs. The wire rope sling had three u-clips where the 7/16 inch wire rope goes through the master link and were clipped on the dead end of the wire rope. The wire rope sling had three u-clips where the 7/16 inch wire rope goes through the metal hooks and were clipped on the live end of the wire rope which could cause sling failure. Feasible abatement methods for reducing this hazard, include but are not limited to the following: 1. Install the u-clips on the dead end of the wire rope and then have the sling proof tested. 2. Use manufactured wire rope double leg slings that are made by a sling manufacture.
