Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Medical Arts Building, 185 North Wabash Avenue, Chicago, Illinois - The freight elevator, number 6, employee(s) were ex- posed to hazard(s) of being caught between the elevator car platform and structural members of the building and/or shaft walls resulting in crushing, amputation and/or death. A struc- tural setback, approximately 10 inches from the edge of the ele- vator platform at the track level, allowed employee(s) riding the elevator to move off the elevator car platform near the ledge at the base of the structural setback. Among other meth- ods, one feasible and acceptable methods(s) to correct this haz- ard is to: (1) ensure that all safety devices are in an operable condition on the car doors at the south end as stipulated by ANSI/ASME A17.1-1987, American National Standard Safety Code for elevators, escalators, and moving walks and ANSI/ASME A17.3- 1986, American National Standard, Safety Code for Existing Ele- vators and Escalators.
