SECTION 5(A)(1) OF THE OCCUPATIONAL SAFETY AND HEALTH ACT OF 1970; THE EMPLOYER DID NOT FURNISH EMPLOYMENT AND A PLACE OF EMPLOYMENT WHICH WERE FREE FROM RECOGNIZED HAZARDS THAT WERE CAUSING OR LIKELY TO CAUSE DEATH OR SERIOUS PHYSICAL HARM TO EMPLOYEES IN THAT: A) WORKERS WERE EXPOSED TO THE HAZARDS OF BURNS, BROKEN BONES, CRUSHING, LACERATIONS, AND DEATH FROM THE UNCONTROLLED OR IMPROPER IGNITION OF THE FUEL AND AIR MIXTURES INVOLVING THE BRYAN CORPORATION STEAM AND BOILERS; THE SUPPLY GAS, AIR, WATER, STEAM, AND ELECTRICAL SERVICE LINES; AND THE STEAM BOILER CONTROL SYSTEMS. THE HAZARDS CAN BE REMOVED BY A PROGRAM THAT INCLUDES, BUT IS NOT LIMITED TO THE FOLLOWING ELEMENTS: 1) REVIEW THE WRITTEN PROCEDURE FOR START-UP OF FURNACES. THE PROCEDURE SHALL BE USED UPON DOCUMENTED INSTRUCTIONS PROVIDED BY THE INSTALLER AND/OR MANUFACTURER OF THE FURNACE AND ITS CONTROL SYSTEM. THE START-UP PROCEDURE SHALL BE EXPECTED TO INCLUDE ELEMENTS THAT INCLUDE: A) ASSURE THAT ALL OF THE GAS VALVES, ELECTRICAL SWITCHES, AND CONTROLS ARE POSITIONED AS REQUIRED. B) LIGHT THE PILOT LIGHT. C) DETERMINE THAT THE PILOT LIGHT IS BURNING AND THAT THE FLAME IS STABLE. D) OPEN GAS VALVES, SET SWITCHES, AND TAKE APPROPRIATE READINGS IN THE PRESCRIBED ORDER. E) IF ANY DEVIATIONS FROM THE PRESCRIBED START-UP ARE ENCOUNTERED, IMMEDIATELY USE THE DOCUMENTED EMERGENCY SHUTDOWN PROCEDURE AND NOTIFY MANAGEMENT. DO NOT BEGIN THE START-UP PROCEDURE UNTIL GIVEN SPECIFIC INSTRUCTIONS BY DESIGNATED RESPONSIBLE AUTHORITY.
