Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not Furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Rig #3, Richard Barnett unit well J1, employees were subject to the hazards of being struck by and severaly injured should a four inch inside diameter pressurized vibrator hose with an operating pressure of more than 200 psi gauge, become disconnected and whip about striking them. Among other methods, one feasible and acceptable abatement method to correct this hazard is to secure the hose using chains properly located and of sufficient strength to resist hose whipping should it become disconnected or broken, as referenced in paragraph B.14.3 of API-RP 54, "Recommended Practices For Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations."
