Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Building 49 N Column W-17 and W-18. On and/or prior to 10/6/89, an octagon scaffold constructed to fit inside an open ended cylinder for the purpose of supporting worker(s) doing work inside the cylinder was not cross braced or otherwise sufficiently supported to prevent failure and/or collapse of the scaffold. The worker(s) were exposed to serious injury and/or death from a fall of approximately 12' onto a round steel table containing projecting pipes on the pipes on the pit floor below.
