Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by failed crane loads when changing patterns on the molding unit. Patterns weighing several thousand pounds each were suspended from a monorail underhung bolster crane which had at least 15 broken wires in a one foot section of rope lay for two of its floor hoist lines. Among other methods, one feasible and acceptable method of abatement to correct this hazard is to initiate a rope inspection and replacement program such as that outlined in American National Standards Institute B30.11 "Monorail Systems and Underhung Cranes" -1973 which includes, as a minimum, the following elements: 1. Visual inspection of all running ropes once each working day. 2. Thorough inspection of all ropes at least monthly with a full, written, dated and signed report of rope conditions kept on file. 3. Removal from service of ropes with any deterioration resulting in appreciable loss of original strength where further use of the rope would constitute a safety hazard.
