Text For Citation: 01 Item/Group: 001 Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to possible burns and injuries caused by fragments due to possible leakage or rupture of dryer cans on PM #'s 3,4,& 5. These cans are under pressures ranging from 30 PSI to 100 PSI and had not been properly inspected to determine among other things, wall thicknesses, existence of cracking, and conditions of head bolts. One feasible and accepted method to correct this hazard is to implement an unfired pressure vessel program in accordance with TAPPI, 1988, TIS 040216, Paragraph E. A) LOCATION: MACHINE ROOMS THOROUGH INSPECTIONS WERE NOT DONE AND THE SUPPORTING DOCUMENTATION KEPT TO INSURE THAT THE TWO YANKEE DRYER CANS ON PM #3 AND #4 AND THE DRYER CANS ON PM #4 WERE STRUCTURALLY SOUND TO PROHIBIT LEAKAGE AND/OR RUPTURE OF THE PRESSURIZED CANS.
