Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee using improper rigging practices was exposed to a fall hazard because he was unable to adequately evaluate the strength of the anchor points: a) Academy House 1420 Locust Street, Phila, Pa. 19102 - an employee inadequately trained in proper rigging practices when using a single point suspension system fell thirty-eight stories when the wire rope attachment point of his system failed (fatality occurred 12/12/90) Observed 12/12/90 NOTE: One useful and feasible means of abatement, among others, would be to conduct employee training on proper rigging practices for each location prior to starting the job.
