Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion by inverting cylinder while residual LP & Mapp gas was being vented, causing an accumulation of an explosive gas-air mixture/source of ignition open flame burner and/or spark producing tools. Also the employer failed to instruct and/or train employees in the proper procedures for handling cylinders: a) 804 Avon Avenue, Atlanta, GA - Loading Dock on or about October 25, 1988. A feasible and acceptable methods of abatment includes but is not limited to burning off gas through an appliance burn off by flaring in proper location and train employees in the proper handling procedure.
