Text For Citation: 01 Item/Group: 001 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the unexpected release of reels of cable due to the improper installation of winches for lifting applicatins and inadequate equipment inspections: BUILDING 203, TWINNING: WINCHES WERE INSTALLED ON TWINNERS T47, T48, T49, T50, T51 AND T52 TO LIFT REELS OF CABLE. 1.AS PER INFORMATION PROVIDED ON THE WINCH INFORMATION PLATES, THEY WERE NOT SUITABLE FOR LIFTING APPLICATIONS. 2.HOOK ATTACHMENTS WERE NOT PROPERLY INSTALLED ACCORDING TO EITHER MANUFACTURER'S RECOMMENDATIONS OR WIDELY PUBLISHED GUIDELINES. THIMBLES WERE NOT USED, WIRE ROPE CLIPS WERE IMPROPERLY INSTALLED AND HAND SWAGES WERE USED. 3.WIRE ROPE CONTAINED BROKEN AND FRAYED WIRES. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THE HAZARD, AMONG OTHERS, IS TO: 1.REPLACE WINCHES WITH HOISTS THAT WERE DESIGNED AND APPROVED FOR LIFTING APPLICATIONS. 2.INSTALL HOOK ATTACHMENT IN ACCORDANCE WITH MANUFACTURER'S GUIDELINES OR OTHER WIDELY ACCEPTED GUIDELINES. 3.DEVELOP AND IMPLEMENT A SCHEDULE OF REGULAR, DOCUMENTED INSPECTIONS FOR LIFTING EQUIPMENT.S
