Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was f from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling out of and/or being struck by a manually elevated work platform when during horizontal relocation an employee was allowed to ride on the platform, the area was not checked for firm footing, location of personnel and debris, and other obstructions and the platform was not lowered to base level. a) On 4/28/92 , Job site - lower parking level, northern sector Employees used a manual roll around lift (High horse model XK-1, Serial #80039) at an approximate height of 12 feet to chip concrete off the ceiling. One employee was on the platform and two at the base of the lift moving it to new locations as required. During one horizontal relocation the lift was pushed up an inclined surface and overturned. Employees exposed to falling approximately 12 feet to asphalt surface and/or being struck by falling platform. Among other methods, one feasible and acceptable abatement method to correct this hazard is to, during horizontal relocation, 1. Lower the platform to base level. 2. Allow no personnel to ride on the platform. 3. Check area for firm footing, location of personnel and debris and other obstructions. {As per ANSI A92.3-1980 Section 13.3.7 sub-sections (1), (3) and (4) }
