Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 04/04/02 for employees engaged in the manufacture of pre-stress concrete bridge supports and exposed to the potential hazard of being struck by 0.60 diameter cable being stressed to approximately 13,400 lb/sq. ft. of upward force. Among other methods, three combined feasible and acceptable abatement methods to correct this hazard would be to: 1. Replace the existing clevis now in use with a clevis engineered and designed to withstand the upward force imposed by the drape strand and potential cold brittle fracture created by cold weather use. 2. Engineer a system for the pre-stress concrete pouring process that would eliminate the need to install a wood shim under the clevises holding the drape strands. The elimination of the wood shim would remove employees from a "line of fire" hazard created by the drape strand cable. 3. Engineer and install a system of restraints that would be applied to the drape strands and all other cables under tension in the pre-stress concrete pouring process to contain those cables under tension in the event of cable or clevis failure.
