Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of flammable and/or toxic atmospheres and explosion and/or fire inside and outside a petroleum storage tank: (a) On or about January 29, 1992, employees performed cleaning and tank removal at 477 South Schuyler, Bradley, Illinois. Proper grounding and bonding procedures were not utilized when inerting, potential sources of ignition were not eliminated, all lines, except vent lines, were not removed, and flammable vapors were not discharged from the tanks at a minimum height of 12 feet above grade during the inerting process. Among other methods, one feasible and acceptable method to correct these hazards is to establish the requirements of the Illinois Administrative Code, Title 41, Section 170.670 and the requirements of the American Petroleum Institute Recommended Practice 1604 and incorporate these into the company's operating and training procedures.
