Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: JP#1 Oilwell, employees servicing the oilwell removed and re-installed the horsehead on the Lufkin pumping unit, designation number M-228D-213-86, Series J, Serial Number C-67080-J-410956, without securing the horsehead to the walking beam, exposing them to the hazard of being struck by a falling horsehead. Among other methods that can be used for safe installation of horsehead one feasible and acceptable abatement method to correct this hazard is securing the horsehead to the walking beam using the bolt provided by the manufacturer and following the manufacturer's instructions.
