29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), work practices that were used were not suitable for the conditions under which the work was performed: a.) American Refining Group, Inc. - On or about January 28, 2015 and at times prior thereto, safety-related work practices including, but not limited to, the use of appropriate electrically rated personal protective equipment, was not required and in use during installation of an Allen Bradley brand 480 Volt starter. The 480V starter was being installed into the Crude Unit Motor Control Center which was not de-energized prior to installation of the motor starter.
