Text For Citation: 01 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to moving parts by unintended operations of equipment: a) C-Line, Potato Bin Area, potato bin equipment, a positive meansa to lock out power for a rotating screw conveyor in the bin was not provided to protect employees against unintended operation when performing work inside of the bin, March 4, 1986. b) A and B Lines, A and B potato feeder equipment, a positive lock out of power for the rotating screw conveyor at lower position of hopper feed was not provided to protect employees against unintended operation when performing work inside of the hopper, March 4, 1986. Among other methods, a feasible and acceptable abatement method to correct this hazards is: 1) To provide Zero Mechanical State (ZMS), as to provide maximal protection against unexpected mechanical movement of equipment by locking out the electrical and mechanical power to the equipment. In addition, a written program for ZMS should be established with training of all affected personnel.
