Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about January 19, 1987, at U.S. Foundry and Manfacturing Corp. in the Loop Building, at the punch out area, employees were permitted to work in the point of operation of a hydraulically operated, coating punchout machine, without the punch out frame being blocked or secured against falling (sliding), exposing the employees to the hazard of being struck by, or caught in and between the falling punch out frame and the anvil. Among other methods, one feasible and acceptable abatement method to correct this hazard is to require the use of safety blocks or other restraining devices before service or repairs are made on or in the point of operation of machines, and to establish lock out and enforce procedures for the servicing and maintenance of equipment.
